INSURANCE INFORMATION
Vineyard Medical Care is contracted with most major insurance companies, please see list below. If your insurer is not listed, or you are unsure whether they are associated with any of the below companies, you may inquire with our office at 508-693-4400 or our billing department at 800-787-1596.
If you are uninsured, or we are not contracted with your insurer, you may still be seen for an office visit at a flat rate of $180* payable at the time of your visit. This fee will cover all Vineyard Medical Care services provided at the facility, including any on-site laboratory testing**.
After your visit you may request a detailed receipt that can be submitted to your insurance provider.
*Excluding: DOT, Coast Guard, Police, Employment, and Sports/School Physicals.
**Any off-site testing is not included and billed separately by Quest Diagnostics.

HMO plans: If you are not an established primary care patient of Vineyard Medical Care, you are required to obtain a referral from your primary care provider for your visit. If you do not obtain the referral, you will be responsible for any charges of services rendered.
